Dr. Cox unexpectedly reappeared in the second-to-last episode of the new Scrubs season, and the show then revealed some very sad news about his health.

In the first episode of the Scrubs revival, John C. McGinley’s character, Dr. Perry Cox, resigned from his position as Chief of Medicine at Sacred Heart Hospital.

As he departed, he passed leadership to his former protégé, John ‘JD’ Dorian (Zach Braff), effectively launching the new show.

However, Cox reappeared in the second-to-last episode of the season, ‘My Odds,’ under heartbreaking and surprising circumstances. Be warned: spoilers follow…

Dr Cox is now a patient in the Scrubs revival

At the beginning of the latest episode, Cox initially seems to be just checking up on JD. However, his repeated visits make JD suspicious, and that’s when JD passes out.

It seems Perry has an autoimmune condition that doctors can treat, and he’s currently receiving care at Sacred Heart.

According to McGinley, Michael C. Hall occasionally reveals a vulnerable side, and episode 108 allowed him to really explore that emotional depth. He described the scene as beautifully written and a joy to perform, showing a glimpse of the character’s hidden heart.

We spent a lot of time carefully crafting the scene with Sarah Chalke, focusing on how her character helped him address and make amends for some serious errors he’d made as a teacher.

Okay, seriously, that scene with Zachy in the third act just gets me. When my character says, ‘Yeah, I want you to be my doctor, I was trying to protect you from this but yes, you’re what I wrought. Now it’s time,’ it’s so intense! It’s like, finally admitting everything and taking responsibility, but also realizing things have gone too far. It’s a really powerful moment, and I love how it plays out.

I naturally became emotional while filming, between the start and end of the scene, as my character would have. It wasn’t a surprise, but it made me realize I’d been holding onto these feelings for a while.

John C. McGinley believes Cox will be back “a lot more” in Season 2

During a conversation with Deadline, McGinley shared his thoughts on Perry’s possible return for a second season, suggesting that John Cox’s character will likely have a larger role. He also mentioned the positive fan reaction, which he found encouraging.

I see a clear story structure at play here, similar to the way Joseph Campbell analyzed myths. Zach’s character functions as the hero in a classic three-act story, and like any good hero, he needs real challenges and something at stake. Without strong antagonists like Ken Jenkins’ character or even enough of the Janitor, the main threat to Zach and the interns this season becomes the possibility of failure, which works well as a central conflict.

He needs real stakes and consequences. There has to be a sense of danger, and that danger will come from the character of Cox, however the showrunner, Billy Lawrence, portrays him. I expect Cox to play a much bigger role in Season 2.
