To get this item, you’ll first need to finish two side quests: “One Gassy Issue” and “One Slimy Issue.” You’ll receive these quests from Beth in the Fades District of Dominion, but they won’t become available until you’ve completed Main Mission 18: Means of Ascent. Your best chance to actually get the item is during the fight with the Bio-Thresher Omega boss, which is unlocked after finishing those quests.
