Elden Ring Millicent Questline All Steps and Rewards

Anyone who’s played Elden Ring for a while knows that the characters you meet usually need your help with something. Millicent’s story, which takes you from the diseased swamps of Caelid to the majestic Haligtree, is one of the most moving and beneficial quests in the game. It’s almost essential if you’re building a character focused on quick attacks and Dexterity, or if you’ve been afflicted by the Frenzied Flame and are looking for a different ending.

How to Start the Quest in Caelid

To start this part of the game, travel to eastern Caelid. Look for Gowry’s Shack, a small wooden building on the road to Sellia, Town of Sorcery. Inside, you’ll meet Sage Gowry, an old man trying to save Millicent, a girl who’s suffering from scarlet rot at a nearby church.

He’ll ask you to get an Unalloyed Gold Needle from a boss in the nearby swamp. Luckily, the latest update (1.12) makes it much easier to track items like this. When you pick it up, it’ll appear in your recent items list with an exclamation mark, so you won’t have to search through your whole inventory to confirm you have it.

Beating Commander O’Neil and the Unalloyed Gold Needle

Okay, so you’re looking for that needle? It’s held by Commander O’Neil, and let me tell you, getting to him is tough! He’s deep in the Swamp of Aeonia, and always has those spooky ghost soldiers around. I’ve battled him with a few different characters now, and the biggest thing I’ve learned is to use the arena to your advantage. There are these scarlet rot geysers everywhere, and if you can lure him over them, they’ll really do a number on his health!

After you defeat the enemy, they’ll drop the Unalloyed Gold Needle. It’s broken, so you need to bring it back to Gowry. He’ll take some time to repair it – you can wait by resting at a Site of Grace or by reloading the area. Once you speak to him again, he’ll give you the fixed needle and a note called Sellia’s Secret.

How to Find the Three Beacons in Sellia

Getting to Millicent usually requires passing through Sellia, the Town of Sorcery. However, the town is magically sealed. Luckily, Gowry gave you a note detailing how to break the seal – you need to light three flames on top of the candle towers you’ll find on the rooftops.

  • The first tower is on the southwest side, near the main entrance.
  • The second is in the center of the town, reachable by jumping across the roofs.
  • The third is on the north side, near the cliff edge.

Lighting these special beacons unlocks the path leading up the hill to the Church of the Plague. It’s similar to defeating bosses in Elden Ring – you need to overcome a challenge to progress.

Curing Millicent and Getting Your First Reward

You’ll find Millicent slumped against a wall at the Church of the Plague, and she’s hurting. When you give her the needle, she’ll accidentally prick herself with it and faint. After resting at the Site of Grace and talking to her until she wakes up, she’ll reward you with the Prosthesis-Wearer Heirloom. This charm is especially helpful when you’re starting out, as it boosts your Dexterity by a solid +5.

Following these events, Millicent sets off on her own path. You can briefly find her again at Gowry’s Shack in Caelid before she travels north. It’s worth speaking with her there, as it provides valuable insight into how she recovers her memories.

Moving North to Altus Plateau and the Valkyrie’s Prosthesis

Okay, so I finally made it to the Altus Plateau, and I spotted Millicent chilling near the Erdtree-Gazing Hill grace. She told me about her missing arm, which is really messing with her combat. Apparently, to help her out, I need to trek all the way to the Shaded Castle – it’s hidden in this nasty, poison-filled valley up north. Sounds like a quest!

You’re looking for the Valkyrie’s Prosthesis. You can find it inside a chest in a room on the northwest side of the castle, but a Cleanrot Knight is guarding it. While you’re in that area, be sure to collect any golden seeds you find – the enemies there are very strong, and you’ll need extra healing flasks.

The Godskin Apostle and Windmill Village

Once you’ve given Millicent her new arm, she’ll head to Windmill Village, which is located in the northeastern part of the Altus Plateau. Be warned: this village is unsettling and populated by hostile villagers who will immediately attack you. You’ll face a tough battle against a Godskin Apostle at the top of the hill.

Okay, so if you’ve been following Millicent’s whole storyline, you can actually summon her for this boss fight, which is awesome! Once you beat the boss, make sure to rest at the Site of Grace that appears. Talking to her then is a really big moment – she’s starting to understand her connection to Malenia. It’s kinda sad but cool, because she basically represents the part of Malenia that got lost when she fought Radahn – like, her honor and sense of self. It’s a huge lore drop!

The Final Stretch in Miquella’s Haligtree

Next, travel to the Mountaintops of the Giants and find the Site of Grace at the Ancient Snow Valley Ruins. Speak with her there, and then get ready for a challenging journey to the Haligtree. You’ll need to find both pieces of the Haligtree Secret Medallion and solve a puzzle in Ordina, Liturgical Town to proceed.

You can find Millicent praying in the Haligtree’s Prayer Room. After talking to her, go down to the Drainage Channel. There, you’ll encounter a large pit filled with scarlet rot, guarded by a Putrid Tree Spirit. Defeating the Spirit is essential to continue Millicent’s quest.

The Big Decision: Help or Betray Millicent

After defeating the tree spirit, you’ll find two summoning signs on the hill. This is where the quest branches, and you’ll need to choose a path.

  • The Gold Sign: You help Millicent fight off her four sisters. This is the “good” ending that rewards you with the Rotten Winged Sword Insignia.
  • The Red Sign: You invade Millicent and kill her yourself. This rewards you with Millicent’s Prosthesis immediately.

If you decide to help her, you’ll find her resting near the rot pool. Be careful not to jump on her – the game’s design in that area is a bit strange, and you could accidentally kill her, which would stop you from completing the quest. Once she passes away peacefully, you can collect the Unalloyed Gold Needle from her remains.

Successive Attack Multipliers and Multiplicative Stacking

Here’s where things get exciting for players who enjoy optimizing their builds. The talismans earned through this quest work with a unique system I call AccumuVal. Basically, the more hits you land, the more damage you deal – it increases in stages as you reach certain point totals.

  • Winged Sword Insignia: 3%, 5%, and 10% damage boosts.
  • Millicent’s Prosthesis: 4%, 6%, and 11% damage boosts, plus that +5 Dexterity.
  • Rotten Winged Sword Insignia: 6%, 8%, and 13% damage boosts.

To deal a lot more damage, combine Millicent’s Prosthesis with either version of the Winged Sword Insignia. Because the game calculates damage increases by multiplying them together, using the Prosthesis with the Rotten Winged Sword Insignia can increase your damage by more than 25% when your weapon is fully upgraded. This bonus lasts as long as you keep attacking, but it will start to fade after about 1.5 seconds of not attacking.

Using Miquella’s Needle to Change Your Ending

If you’ve already helped Millicent and recovered the needle, there’s one more step. You’ll need to defeat Malenia, the Blade of Miquella. Once you win, a large red flower will bloom in the arena. Using the needle on this flower transforms it into Miquella’s Needle and rewards you with a Somber Ancient Dragon Smithing Stone. These stones are very rare – there are only 14 to find in the whole game – making this a valuable item to obtain.

If you want to undo the Flame of Frenzy ending, you’ll need Miquella’s Needle and take it to the heart of the storm in Farum Azula – specifically, the arena where you battled Dragonlord Placidusax. But even if you’re happy with your current ending, the game’s story shows that Gowry wasn’t truly helping Millicent. He was a collective of Rot creatures who wanted her to become a valkyrie for his own purposes. By assisting her, you ultimately allow her to die on her own terms, preserving her agency.
