
Caroline’s quest to save her sister in Tormented Souls 2 is packed with intriguing backstory, challenging puzzles, and lots of items to find. Given everything the game offers, it’s no surprise that the survival horror sequel features a wide range of trophies and achievements.
It’s true that some of the puzzles in the game can be quite difficult, especially for players unfamiliar with this style of horror. Here’s a guide to unlocking every trophy and achievement in Tormented Souls 2.
All Trophies & Achievements in Tormented Souls 2 (How to Unlock)

This trophy and achievement guide includes major spoilers about the game’s story, including details about the different endings. Please proceed with caution if you haven’t finished the game.
Tormented Souls 2 has 33 achievements and 34 trophies (including a Platinum trophy) to collect. Getting all of them will require playing the game several times, especially the ones tied to different difficulty levels, as you can’t change the difficulty once you start. However, you can unlock all the different endings – and the achievements that come with them – in a single playthrough if you save strategically and complete the necessary tasks for each ending when they become available.

| Registry | Save the game | Use the recorder in any safe room to save your progress by using a Recording Tape item on the machine and pressing play. Caroline will recount the most recent events to herself (if applicable; sometimes she won’t talk) and then the save will be complete. If you’re familiar with saves in classic Resident Evil games, it functions much the same way, meaning that the recorder is the only way your progress is saved, so be mindful. The one exception to this is if you play on Assisted difficulty, which includes autosave. Note that this trophy/achievement only seems to pop by overwriting an existing save. |
| Fashion | Change outfit | Interact with the white wardrobe in Caroline’s guest room to open it and select the outfit you’d like to change her into. Note that you can only do this at the start of your first playthrough if you’ve purchased the Digital Deluxe Edition that includes bonus outfits for Caroline. Otherwise, you’ll need to complete at least one qualifying playthrough to unlock those respective outfits. |
| Death | Let the shadows consume you | Walk into any dark, shadowy area and stand there until you die. You’ll know it’s a dangerous area if your screen starts to distort and foreboding music plays. This can be achieved within the first hour of the game. |
| Femme Fatale | Create the nailer | The Nailer is the first weapon you can obtain in the game, and you’ll find the parts for it inside the Workshop room. Collect the Nailer Frame and Air Canister there and then assemble it via your inventory. You need boxes of Nails to use as ammo for this weapon, which you can collect while running around the villa. |
| Amnesia | Complete the game without saving (Unavailable on Assisted difficulty) | This trophy/achievement can only be unlocked on Standard difficulty or higher, where autosaving is not present. At no point can you save your game, be it an autosave or a manual save via a recorder in a safe room. |
| Anxiety | Complete the game in under five hours | This is the quintessential speedrun trophy/achievement of the game, and playing through the story at least once before attempting this is recommended. You can also consider doing it on Assisted difficulty, to make combat easier and quicker to get through. |
| Crucifixtion | Kill an enemy using a trap | An easy way to get this early in the game is via the rotating spike devices in the Prison area. Essentially, lure an enemy toward an environmental hazard and kill them with it. |
| Futile | Complete the game with the bad ending | To achieve the game’s “bad” ending, you’ll need to head directly to the Bunker area after the School and Church areas. This means you won’t save Miguel from death back at the Convent, and subsequently, he won’t be able to help save Caroline and Anna before the encounter at the Temple of Nautilus. Note that this is the ending you’re likely to get off-handed the first time, as the good ending requires you to take a heavy detour to complete various objectives to help Miguel live. |
| Gunsmith | Upgrade all weapons | Certain weapons will have upgrades that you can apply yourself via your inventory menu (e.g. — pipes that can increase the number of barrels for your shotgun). Find and apply upgrades to all qualifying weapons in the game. |
| Hoplophile | Collect all weapons | To achieve this, you need to collect all variations of the Nailgun, the Shotgun, the Crowbar, the Crossbow, the Sledgehammer, the Hand Cannon, the Chainsaw, and the Electric Lance. |

These are diaries, and several of the game’s major NPCs have a series of diaries to collect from each major area. | Warlock | Collect all of Joseph’s diary entries | Throughout the game, you’ll come across items that resemble open books with writing and drawings. | Mad | Complete the game on Tormented difficulty | You need to first complete the story on Standard difficulty in order to unlock Tormented difficulty, which is the most challenging one in the game. |
| Master Key | Solve all the combination key puzzles | Complete the three combination key puzzles that are found at the School and Church. |
| Ranger | Complete the game without using the map | Don’t use any map a single time in one entire playthrough. You will need to pick up at least one map in the game (might as well be the first one in the Convent), in order for the game to track your usage (or lack thereof). Just make sure not to use it! |
| Ruthless | Kill 40 enemies | A self-explanatory trophy/achievement; kill a minimum of 80 enemies in a single playthrough. Do your best not to skip any enemies while exploring each area of the game. |
| Serial Killer | Kill 120 enemies | A self-explanatory trophy/achievement; kill a minimum of 80 enemies in a single playthrough. Do your best not to skip any enemies while exploring each area of the game. |
| Survivor | Complete the game on Standard difficulty | As stated, complete one full playthrough of the story on Standard difficulty, which is the game’s default difficulty. If you’re not used to mechanics such as finite supplies to save your game or tank controls/camera angles, you may want to start with Assisted difficulty first to get used to those controls, combat, etc. |
| Untouchable | Complete the game only using one health item | As it implies, you can only use one health item during an entire playthrough of the story. Whether it’s Povidone, Painkillers, or a First Aid Kit, choose wisely when to use it. It’s wisest to chase this trophy/achievement on Assisted difficulty, as you’ll have a certain amount of passive healing for Caroline. |
| Vindication | Complete the game with the good ending | To achieve the game’s “good” ending, you’ll need to do a good bit of backtracking to ensure that Miguel lives to save Caroline and Anna at the Temple of Nautilus. Before you head to the Bunker, go all the way back to the Convent and use the Sledgehammer on the crumbling brick wall in the Refectory room. Get the Duct Tape from there, then head back to the School. Enter the Other Side via the mirror in the girl’s bathroom, head to the school director’s office, and use the Duct Tape on the robotic figure in the room that represents Miguel. This endeavor saves his life and thus allows him to help Caroline and Anna before the final encounter. |
| Walker | Complete the game on Assisted difficulty | As stated, complete the game on Assisted difficulty, which is essentially the game’s “story mode” difficulty. It features autosaving each time you enter a different area, along with the manual recorder saves. Combat is substantially easier, consumables (e.g. — ammo, health, recorder tapes) are more plentiful, and Caroline’s health will passively recover to “Caution” status if she reaches “Danger” status. If you’re new to tank control gameplay and these type of manual saves, this difficulty may be the best to start with. |
| Escape | Escape from the infirmary | When Caroline wakes up in the villa’s infirmary after trying to rescue Anna at the start of the game, once she’s off the bed, gather any important items you can from the room and then exit to the hallway outside. The trophy/achievement will automatically unlock at that point. |
| Reflection | Enter the Other Side using a mirror | Complete the Teleport Mirror puzzle inside the southmost room of the Torture Museum to unlock its power. After a cutscene, Caroline will emerge in the Other Side, which is an alternate, hellish version of the area around you. The trophy/achievement will automatically unlock. |
| Journey | Travel to the past | Use the projector in the Conference Room of the Processing Plant (via the VHS tape) to travel to the past in order to block a vent with a wardrobe and save Anna from a grisly demise. |
| Blasphemous | Defeat the Iron Maiden | Gain access to the Chapel in the convent and defeat the game’s first boss. While dealing damage to the boss itself, you also need to obtain a dagger from the two corpses hanging on opposite walls in the room. Once the boss falls to its needs and opens its helmet, shove a dagger into one of its eyes. Repeat the process until you can shove the other dagger into the boss’s other eye, ending the fight. |
| Avenger | Defeat the Harvester | You’ll encounter the Harvester boss in the Slaughter Room area of the Processing Plant. You need to use the Harpoon gun machine in the room against the boss twice. Load the gun with the harpoons found on the wall opposite it, then use the crank on the other side of the room to pressurize the gun. Once it’s pressurized, use the small control panel on the opposite side to fire the harpoon when the boss is in front of it. Use your weapons to deal enough damage to temporarily stun the boss for several seconds to achieve this. |
| Butcher | Defeat Pighead | Defeat the Pighead boss in the School area. While damaging the boss with your weapon(s) of choice, use the electrical machines on each side of the arena to damage and stun him for a few seconds. Once you’ve done this (and enough damage overall) with each electrical machine in the room, Pighead will be defeated, automatically unlocking the trophy/achievement. |
| Undertaker | Defeat Dr. Hertz | Defeat the Dr. Hertz boss in the Cemetery area, after the cutscene sends you plummeting into a mass grave within the Other Side. The boss has a coffin on its back, much like a turtle with a shell. Your goal is to shoot the boss from its front, where it’s most exposed, to send it toppling onto its back. When that happens, use your most powerful weapons to deal damage until it gets back up. Repeat this process until it’s dead, while dealing with the zombie minions that appear periodically. |
